Advance Care Planning: It’s About the Conversation
3 part series August 29, September 5 and 12
What if a sudden illness or injury left you unable to speak for yourself? Who would you want to speak for you? Advance care planning is a process that helps you:
Think about your health care values and goals;
Consider health care choices you may have to make in the future;
Talk about your choices with your doctor and your loved ones; and
Make a written plan for the future (advance directive): Copies of an Advance Care Planning Document will be available
Many advance directive documents are filled with confusing language, distracting us from important considerations in our planning.
Join Pema to delve into some of these important questions:
Session 1: What is an advance care plan? What factors should I consider in choosing an agent? What do I need to do before I fill out the documents?
Session 2: How do my spiritual beliefs affect my decisions about care? What questions should I ask myself as part of this planning process?
Session 3: How do I fill out the document so that my wishes are clear and actionable? How do I prepare the document for signatures?
Sessions 1 and 3 are focused on the process of advance care planning. The discussion in Session 2 is focused on spiritual questions suitable for those with any or no faith tradition, with clarifying examples from Buddhist practitioners.
